Director of Professional and Clinical Services MINIMUM QUALIFICATIONS Has previous word processing experience, preferably in home health or similar operation.
Able to use a variety of software applications.
Computer proficiency.
Prefer a minimum of one-year practical experience, or graduation from a one or two year business college.

Prefer ability to type at least 50 words per minute.
Has ability to use personal computer, copier, scanner and facsimile machine.
Knowledge of medical terminology preferred.
Highly attentive to details and accuracy.

SUMMARY OF JOB RESPONSIBILITIES The Intake Coordinator is responsible for central intake data entry and assuring that all client referrals are processed and managed in an accurate, timely, coordinated and efficient manner.
RESPONSIBILITIES AND DUTIES Promotes a positive work environment by consistently interpreting agency philosophy, mission, values, and standards of care, and providing a professional role model for other staff.
Answers and tracks all calls to the Referral Line, speaking knowledgably about home health services, processes referral timely, and gives information to callers or routes calls to appropriate staff.
Performs Referral entry into HCHB.
Ensures all Referrals have appropriate documentation including ensuring all Face to Face meet Medicare guidelines coordinating as needed with office DON.
Runs required daily reports, ensuring correct documentation on all referrals.
Coordinates daily with Physician Liaisons to direct duties and track referrals.
Coordinates daily as needed, and hold weekly meetings with all Admission Coordinators in HCHB to assist in coordinating admit for any pending referrals.
Maintains Non-Admit and Pending Binders for all offices in HCHB.
Makes Discharge Patient calls weekly and completes survey following discharge process protocol.
Makes SOC Patient calls and completes surveys weekly.
Meets weekly with all offices as needed, coordinating with DON, AC, and MRS to ensure timely return of all orders and 485's.
Call all patients with a change in payor source from Medicare to HMO, and coordinates with AC to assist patient if interested.
Sends birthday cards monthly to patients and discharge patients in HCHB.
Sends thank you cards as needed to referral sources.
Facilitates contracts as needed with Private Pay, VA, and Pediatrics.
Ensures that back up Intake person is aware and competent to process any pending patients if not in office.
Assists personnel with other duties as assigned by Director of Professional and Clinical Services.
Assist in fostering relationships with potential referral sources identified on the referral line.

WORKING CONDITIONS General office with prolonged periods of computer usage.
Frequent telephone activity.
This position has been designated Class III office position (see Infection/Exposure Control Plan).
Employees performing Class III office position duties are not at risk for exposure to blood borne pathogens and other potentially infectious materials.
All Class III office position employees will not be offered Hepatitis B vaccination.
PHYSICAL REQUIREMENTS Visual/hearing ability sufficient to comprehend written/verbal communication.
Ability to perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing.
Ability to effectively deal with stress.
CONTINUING EDUCATION REQUIREMENTS Agency personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor.
In addition, agency personnel are expected to accept personal responsibility for other educational activities to enhance job related skills and abilities.
All agency personnel must attend mandatory educational programs.

Estimated Salary: $20 to $28 per hour based on qualifications.
